HOW TO APPLY FOR PHILIPPINE PASSPORT FOR INFANT IN CEBU

Philippine Passport Applcation for Infant in the Philippines are processed in all DFA (Department of Foreign Affairs) office nationwide.  There are total of 36 site office located in the country which are geographically situated in known cities like Manila, Cagayan De Oro, Cebu, Davao, Bacolod, Iloilo and many more. Online appointment is no longer needed since we are applying for minor which fall under the category of seven (7) years old and below with accompanied parents. Below we provide the 4-step process on the Philippine Passport application for infant which is held at all Department of Foreign Affairs office nationwide.

STEPS ON PHILIPPINE PASSPORT APPLICATION FOR INFANT IN PHILIPPINES

  3. Inside The Embassy
  4. Passport Collection

STEP 1: PREPARE REQUIRED DOCUMENTS

A. Accomplished Application Form (click here for Application Form).

B. Personal Appearance of minor applicant and either parent or authorized adult companion

C. Philippine Statistics Authority (PSA) Birth Certificate.

D. Marriage Certificate is required if only one parent is accompanying the child.

  • If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child.
  • If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate.

E. ​Passport or any Valid Government issued ID of either parents.

In case applicant has No PSA Birth Certificate/Report of Birth yet

  • If born in the Philippines: Certified True Copy of Local Civil Registrar (LCR) Birth Certificate authenticated by PSA
  • If born abroad: original copy of Report of Birth or first endorsement from Consular Records Division

For our case, since we process the passport application and knowing we do not have yet the copy of the Philippine Statistics Authority Birth Certificate we use the Certified True Copy of Live Birth for this application.

STEP 2: APPOINTMENT DAY

We went directly to DFA Consular Cebu office to apply for Philippine passport which is located at Pacific Mall, Mandaue City. We recommend to visit the office in the morning considering we are bringing an infant which is quite difficult.

Monday-Friday (9:00am-5:00pm)
Saturday (9:00am-3:00pm)
Closed on Sundays and Public Holidays

Upon our arrival, we approached the security guard outside the consular office and ask for the application form, which he handed over to us. We filled it out with all the necessary information and pass it back to the security guard in charge. The security guard handed over the application form to the person assigned for assessment in which we are informed to sit down at the courtesy lane and wait for our name to be called.

Note: Wearing of sleeveless/slippers are allowed. It is advisable to wear a shirt with collar and to remove all jewelry during photo taking.

STEP 3: INSIDE THE EMBASSY

We entered the DFA consular office after our name was called. There we lined up under the courtesy lane and waited for our turn to submit the requirements at the counter. After the teller’s verification, we proceeded to the payment section.

After verification, we proceed to the payment section and was provided on the passport application fee.

Application Fee:
Regular
P 950.00 (12 business days)

Expedite
P 1,200 (7 business days)

Since the price difference is not much from Expedite to Regular we choose to choose the Expedite option for our Philippine passport application for infant.  The delivery fee not included and may vary upon the location. Additional convenience fee for online e-payments may be charged per applicant.

Note: Minors which fall under the category of seven (7) years old and below passport validity is good for 5 years.

After we received our payment receipts, we went to another counter for the photo taking. Once done we are advise to keep the collection form and present it upon the collection of the passport.
The whole process application took almost 2 hours to complete which is way convenient as compare to before.

STEP 4: PASSPORT COLLECTION

Before our passport collection we contacted DFA Cebu for the availability status of our Philippine passport application for infant. We managed to e-mail them and was able to received a reply notifying that the passport is now available for collection. Alternatively DFA Cebu consular office hotline is at (032) 520 5898. So we prepare our passport collection stub and receipt prior to our visit to the office

For collection of Philippine Passport Application for Infant, it is not necessarily to bring along the infant, a parent  the parent will just need to be present and collect the passport.
